Refer Your Child To Our Program

As a rehabilitation clinic for children with special needs, there are some boundaries to the program that help define who is appropriate for therapy services at our clinic. Provided below is basic information that will help you determine if your child would be a good fit for the program.  As always, please feel free to contact TherAplay if you have further questions about therapy or would like to request a referral form.

Minimum Age:  18 months

Maximum Age: 13 years

Maximum Weight: Approximately 80-100 lbs, but based on each therapist.  If your child cannot sit independently and requires assistance, the maximum weight is approximately 65 lbs.

Physical Referral

Each patient must have a physician's referral (prescription) for physical or occupational therapy.  The prescription Should Not say "equine therapy" or "hippotherapy" as the movement of the horse is just one of the tools we utilize.

Insurance Information

TherAplay is a provider in Indiana Medicaid. We are also an in-network provider in the following private insurance networks:

• Anthem
• UnitedHealthcare
• Sagamore
• Cigna
• St. Vincent CMO
• Indiana Teamsters

Each insurance plan can vary, so you should contact your insurance carrier to see what benefits your plan provides for outpatient physical and/or occupational therapy.

If your insurance benefits have been maximized, there are options for self-pay and TherAplay's Scholarship program.  This is based on income and number of members in the family.  Further information on taxes, insurance and family will be required.
