Important Treatment Deadline

Like most medical practices, we update our patient files at the beginning of each year to ensure legal compliance and the very best care for your child.

The following must be received by 6 p.m. Tuesday, January 16, 2018 to ensure continuation of treatment at Children’s TherAplay.

  • The completed 2018 Patient Information Update packet must be completed in full and returned to the front desk, emailed to (not your therapist), or faxed to (317) 872-3234.
  • All the patient’s insurance cards (primary, secondary, Medicaid, etc.) must be presented at the Children’s TherAplay front desk for Xeroxing and verification.

Haven’t received your new insurance card yet? Check page three in the Update form to learn how to secure the required information. 

Please note

If these completed packet is not received by 6 p.m. on Tuesday, January 16, the patient’s appointments will be given to a child on the waiting list. Those patients who have forfeited their place on the schedule but would like to resume treatment will be added to the end of the waiting list.
